Forms & Agreements

Intuit has contracted with ICon Professional Services to provide Consolidating Billing Services for classified ICs (Independent Contractors) and Suppliers. All classified ICs and Suppliers will enjoy expedient invoice processing through ICon’s Consolidated Billing Service. To receive consistent and timely payments, ICs and Suppliers are required to complete the following Forms and Agreements.

Complete and fax all forms directly to ICon at (650) 378-4159:

• Background Release
• Direct Deposit Authorization (IC)
